Photo ID
Your passport or UK driving licence. The name on your ID is the name that appears on the certificate — check it matches your visa application exactly.
Your medication list
Anything you take regularly, including the dose. Ongoing conditions and prescriptions do not stop a certificate being issued — the GP simply needs an accurate picture.
Your consulate appointment date
The certificate is valid for 90 days from the date the GP signs it. Knowing your date helps you time the order — and the apostille — correctly.

Complete the online health questionnaire
A short, secure form that gives our GPs everything they need to assess your case. You'll be asked about:
- Your personal details, exactly as they appear on your passport
- Medical history and any current health conditions
- Current medications and doses
- Recent symptoms or infections
- Vaccination history, where the destination requires it
The questions follow the International Health Regulations (IHR 2005) and the documentation consulates typically request.
Review by a UK-registered GP
Your questionnaire goes to a GMC-registered General Practitioner who is also registered with the Foreign, Commonwealth & Development Office. They assess whether the certificate can be issued safely and appropriately.
If anything needs clarifying, our team contacts you directly — by phone, email or WhatsApp — before the certificate is signed. In most cases no in-person appointment is required.
Certificate issued, signed and stamped
Once the review is complete, your certificate is prepared to the requirements of the specific consulate handling your application — not a generic template. It is:
- Issued on clinic letterhead with the doctor's GMC number
- Signed and stamped by an FCDO-registered UK doctor
- Worded to confirm you are free from diseases posing a public health risk, per IHR 2005
- Dated — validity runs 90 days from this date

Your signed PDF arrives by email within 24–48 hours of the consultation. The wet-signature original follows by Royal Mail 1st Class, posted the same day at no extra cost — most consulates want to see the physical document, so we send it as standard rather than on request.

Apostille legalisation and sworn translation
Most consulates require your certificate to carry a Hague Apostille, and Spanish applications generally need a sworn Spanish translation. This is the final step before you submit.

Can my own NHS GP issue this instead?
Your GP can write a letter, but apostille legalisation requires a signature from a doctor registered with the FCDO — a separate registration from the GMC that most NHS practices do not hold. Our doctors hold both, so your certificate can be legalised and submitted without difficulty.
Can I still obtain a certificate with an existing health condition?
In almost all cases, yes. The certificate confirms you are free from diseases carrying public health implications under IHR 2005 — it is not a general fitness assessment. Ongoing conditions and regular medication are common among applicants. Please complete the questionnaire fully so the GP can assess your case accurately.
When should I order relative to my consulate appointment?
The certificate is valid for 90 days from the date the doctor signs it, and apostille legalisation adds further time. Ordering roughly four to six weeks before your appointment leaves comfortable room for both steps without the certificate expiring.
Do I need a video call or a face-to-face appointment?
In most cases, no. The GP reviews your questionnaire and contacts you only if something needs clarifying. Everything is completed remotely.
Do you handle the apostille and translation for me?
These are provided by our partner, The Apostille Service, at rates negotiated for Healthify patients. We issue the certificate, and you arrange legalisation and translation directly with them. We explain the process and link you straight through on our apostille and translation page.
What happens to my medical information?
Your data is handled under UK GDPR and the Data Protection Act 2018, stored securely, and shared only where necessary to deliver your certificate or comply with a legal obligation. Full detail is in our privacy policy.
Can you advise me on my visa application?
The Healthify issues medical documentation only. We are not immigration advisers and cannot advise on visa eligibility, supporting documents or consulate procedure — for that, please consult a regulated immigration adviser or your consulate directly.
