Medical Certificate

Apply for a Medical Certificate of Good Health (IHR 2005) or Fit To Fly Certificate

Before you begin

Please complete the form below with your personal details and answers to the health questions. Enter your name exactly as it appears in your passport, this is the name that will be printed on your certificate, and consulates compare the two literally.

If you have relevant medical records, you can upload them to support your application. For Greek applications, please also upload your TB evidence and syphilis test result, as we cannot issue the certificate without them.

What happens next

Once you submit, our team reviews your details and a doctor assesses your information. Your appointment is confirmed only when you receive an email from us containing the consultation details and your secure video link. If you have not heard from us within one working day, please check your spam folder before getting in touch.

Appointments are conducted online with a GP by secure video consultation. You will need a device with a camera and your passport to hand, so the doctor can verify your identity.

If the form does not work

Technology occasionally gets in the way, and it should never delay your application. If the form will not submit, or a file will not upload, simply email us at admin@thehealthify.co.uk and we will complete the whole process by email instead; same service, same timescales, no delays whatsoever.

You are also welcome to email us if you would simply rather not use a form.

We will post your Medical Certificate with Royal Mail to this address. We operate and post certificates only within the UK.
Please note:
We will try our best to accommodate same day GP appointments. We will email you the link to attend the video call with the doctor.
If the slot is not available, we will provide you with an alternative time slot for your online GP appointment. In case you are not sure about your availability, you can inform us through email once we send you the appointment link. The duration of the appointment is about 15 minutes.

Easy service to use, I had major issues with trying to get a medical certificate from my GP which led to me then getting one from Healthify. Consultation was very good and quick, they were able to get me a medical certificate sent to an apostille service within 4/5 days of me contacting them.
Only issue I had was that the phone number doesn't seem to go through, so when I had an initial issue with submitting the form, I wasn't able to contact anyone immediately, but I did receive an email back before the end of the day and was able to get my issue sorted.
